A $15 charge for each vaccination is payable with cash or check. The fee may also be billed to Medicare Part B or Medicaid for residents who are enrolled in those programs. Medicare or Medicaid billing requires recipients to show their ID card for the program and a valid driver’s license or other form of photo identification. Flu shots will be given until the county’s vaccine supply is exhausted.

People with access to the Internet can download vaccine information forms and consent forms from www.macombcountymi.gov/publichealth. Completing the consent form in advance can save time at the health centers.
