The DHWP Vital Records office is located at 1151 Taylor (Herman Kiefer Health Complex). Birth records are available for persons born in the city of Detroit ONLY. Certified copies of birth records cost $17. Additional copies (ordered at the same time) are $5. Certified copies of birth records cost $5 for senior citizens ages 65 or older requesting their own birth records. In Michigan, birth records are legal documents, and the only person entitled to a birth record is: 1) the individual named therein (18 years or older with proper identification); or 2) the parents named on the document or the legal guardian, who is required to show proof of guardianship. Identification (e.g., valid driver's license, valid state I.D. or three alternate documents -- social security card, voter's registration card and a utility bill) must be shown. For more information regarding birth or death records, call (313) 876-0417. In addition, parents can get their kids vaccinated at any of the City’s health centers. Please bring your immunization records, picture ID and proof of insurance. Medicaid clients are eligible. The locations are:

Detroit, MI - The Detroit Department of Health and Wellness Promotion (DHWP) Vital Records Division will extend its hours to accommodate parents and guardians needing birth records for school registration. DHWP will also provide immunizations at several health centers.
